Addressing health disparities through quality programs in Michigan

 

To address the health disparity in members being tested for kidney disease, UnitedHealthcare Community Plan of Michigan implemented a Kidney Health Evaluation for Patients with Diabetes (KED) program. This included distributing educational materials to 1,579 Black members, updating KED screening best practices through a comprehensive claims analysis and testing the National Kidney Foundation of Michigan’s new chronic kidney disease dashboard. These collective efforts resulted in a 3.43% improvement in KED rates.
