Pennsylvania Health Center Technology Alliance
Pennsylvania Health Center Technology Alliance (PHCTA) is a non-profit organization dedicated to ensuring FQHCs have the technology and tools needed to provide integrated and comprehensive care. PHCTA is allocating $300K in UHC support to modernize electronic medical records (EMR) within FQHCs across Pennsylvania.
The technology upgrades will improve clinical workflows by maximizing business intelligence and reporting. Anticipated outcomes include enhanced efficiency, quality, patient outcomes, and clinician satisfaction.
Cornerstone Care
Cornerstone Care is a community health center that has provided high-quality, affordable, patient-centered health care to Pennsylvanians for more than four decades. As a network of FQHCs serving southwestern Pennsylvania, it offers family medicine, dental, and vision care in addition to behavioral health, mobile care, and pharmacy services.

Supported Pennsylvania health care initiatives
Cornerstone Care will use this support from UHC to fund three key initiatives:
- Increase outreach efforts and care coordination by placing community health workers in each county it serves.
- Hire a data informaticist who will leverage key insights to help improve care.
- Continue operating its popular HONEY Program, a holistic wellness initiative for students and others in Greene County.
With greater community-based engagement and data intelligence resources, Cornerstone aims to decrease onset of chronic conditions and improve disease management through increased screening, treatment plans, and referrals for environmental risk factors.
With another year of funding for its HONEY Program (Healthy Options, Nutrition, Exercise, and Youth), the FQHC will continue to deepen its partnerships with local schools and community organizations to help teens and adults:
- Choose healthy food options through practical cooking classes and meal planning.
- Move more with customized fitness routines and group exercise.
- Build lasting habits by making healthy living accessible, sustainable, and fun.
- Improve overall health literacy and chronic disease prevention.
Community Health Net
Community Health Net (CHN) is a certified Pennsylvania FQHC that has been fulfilling its mission to make health care easy and flexible for the community since 1985. With eight locations in northwest Pennsylvania, it provides family medicine, pediatric care, behavioral health services, pharmacy services, dental care, telemedicine, and more.

Supported Pennsylvania health care initiatives
Community Health Net will extend its operating hours to make it easier for families with kids in school, patients who work standard business hours, and others in the community to receive health services in Erie County.
Overall, CHN will add 15 hours of patient time during the week by staying open from 8 a.m. to 8 p.m. It also plans to offer both medical and dental services on Saturdays from 10 a.m. to 4 p.m.
Centerville Clinics Inc.
Centerville Clinics Inc. is a non-profit FQHC delivering total health care in southwestern Pennsylvania. With comprehensive medical, dental care, and behavioral health services, it is committed to serving individuals in the community regardless of their insurance status or ability to pay.

Supported Pennsylvania health care initiatives
Centerville will launch the Essential Pantry as a resource to assist patients and community members who are struggling with the costs of food and hygiene products.
By reducing food insecurity and financial stress among those the food pantry serves, the Frederick-based nonprofit aims to:
- Promote better nutrition.
- Increase engagement with care services.
- Improve patient satisfaction.
