Investing in provider connectivity in Colorado

 
 

Reliable access to timely clinical information is foundational to coordinated, whole-person care in Medicaid. When clinicians can see a patient's full picture — medications, lab results, recent visits, discharge summaries — they are better positioned to make informed decisions and identify and close gaps in care.

To strengthen data sharing across care settings, Rocky Mountain Health Plans (RMHP) invested more than $330,000 to help 16 providers establish connectivity to Colorado's statewide Health Information Exchange (HIE), Contexture. RMHP targeted practices serving Health First Colorado members preparing to transition to the Contexture HIE platform, prioritizing providers facing the greatest financial or technical barriers to completing the one-time electronic health record (EHR) integration needed to begin exchanging data.

 
 

Why HIE connectivity matters for Medicaid

Health information exchanges play a central role in improving care coordination by enabling secure, real-time sharing of patient information across providers and between providers and patients. An HIE allows providers, pharmacists, and other care professionals to securely access and share vital clinical information electronically, improving the speed, quality, safety and cost of patient care.1

HIE participation is associated with measurable improvements in utilization and outcomes. One study found that providers connected to HIE systems saw reductions in inpatient admissions and emergency department visits, with the most significant decreases observed among Medicaid populations.2 These findings underscore the value of shared clinical data in reducing avoidable, high-cost care and improving continuity across settings.

For Health First Colorado members in rural communities, access to specialists can be limited and care coordination across settings is often more challenging. Shared access to clinical information helps enable earlier intervention, timely referrals and stronger continuity of care.

Building a more connected health system

RMHP is tracking progress including successful onboarding to the Contexture platform, completion of technical integration, and confirmation of active data exchange. These early milestones represent critical infrastructure being put in place to support coordinated care across Colorado.

As more Health First Colorado providers participate in shared exchange, care teams gain greater visibility across the continuum of care. For providers, this means fewer duplicative tests, more efficient referrals and smoother transitions between settings. For members, stronger connectivity reduces information gaps and supports a more seamless care experience.

More broadly, expanded connectivity strengthens the infrastructure needed to support population health management, quality measurement and value-based care models that rely on timely, complete data. By helping providers overcome barriers to HIE participation, RMHP is advancing a more coordinated, data-driven approach to care in Colorado — reducing fragmentation, improving outcomes and ultimately moving toward a more connected, sustainable health system.
